Case Management Services are offered to adults having psychiatric disabilities who require support line in the community. Case Management provides service coordination to consumers within the context of their own environment, facilitating access to needed support, assessing needs for improved functionality, and monitoring progress in areas identified together as service objectives. Case managers assist their consumers in achieving independence with respect for the individual's self-determined goals and values. All referrals are made through Southern Nevada Child and Adolescent Mental Health Services.
