MISSION STATEMENT
To Provide an alternative to homelessness for individuals suffering from severe
psychiatric disability and
those with co-occurring disorders (substance abuse/psychiatric disability) through community
collaboration by empowering, assisting and supporting their efforts to access entitlements,
housing, employment and link with community based services to enable them to reach and maintain
a state of psychiatric and physical stability.
Value to Community
- Immediate access to psychiatric care and medication.
- Intensive Case Management to assure clients arrive at appointment. Not just a referral.
- Assist client in reconnecting with families to reestablish needed support systems.
- Works closely with Nevada Health Centers to address both mental and physical health issues to
improve outcomes in both.
- Provide needed supportive housing that will help them develop skills necessary to live
independently.
- Links clients and families with community support services.
- Collecting data over a three year period to track outcomes that can not only help in local planning
but we will be able to compare against national data for other projects through out the United
States.
- Clients have an alternative to using emergency rooms for ongoing medical and mental
healthcare.
Requirements for Program
- Those with Serious and Chronic psychiatric disability.
- Homeless with priority given to those from a shelter, street or medical facility.
- Willing to accept needed mental health, medical health and substance abuse treatment services
- Client wants an alternative to living on the streets.
